21 One-Shot Tract Dilatation for Supine Percutaneous Nephrolithotomy (PCNL) in Obese Patients

Abstract Introduction With the rising incidence of both obesity and urolithiasis, need to perform stone surgery in high BMI patients is increasing. We analysed safety efficacy one-shot tract dilatation (OSD) technique obese undergoing supine percutaneous nephrolithotomy (PCNL). Method Retrospective review all PCNLs performed by a single surgeon between January 2014 December 2020. Patient factors (age BMI), complexity procedural characteristics (tract size technique) were collected. Outcomes included access success, length stay, operating duration, inpatient complications. Results Overall 104 93 patients. Forty-four had >/ = 30 kg/m2 with median age 53 years, 35.12 Guy’s score 2. When initial puncture was successful (43 cases (98%)), satisfactory on occasions. Dilatation using balloon dilator (24Ch) 12 OSD 31 (range 16Ch 24Ch, 22Ch). Median stay lower when used (1 day) versus (2 days), P 0.03. time 98 minutes 120 (P 0.10). There no significant difference operative or compared < kg/m2. two Clavien-Dindo Grade I one II complications where used. No transfusions required. Conclusions One shot an effective safe means performing PCNL.
